Here is an example eval workflow that I've created using Google Forms, Sheets, and Apps Script (a coding environment inside of G Suite).
Forms collects basic patient information. Sheets is where you put it all together and combine it with information gathered from the in-person examination. Saving as a .pdf creates your final product, timestamped and all.
Apps Script allows you to create repeatable functions, such as naming of tabs, menus, and cells. It took many hours to learn the language and write the code from scratch.
